About Us

About the HFM-Prevention Council


The HFM Prevention Council is a non-profit agency funded by the New York State Office of Alcoholism and Substance Abuse Services.

The mission of the HFM Prevention Council is to increase the health and wellness in Hamilton, Fulton and Montgomery Counties by preventing and reducing addiction disorders through education, recovery supports, supportive housing and community outreach.


The Hamilton, Fulton and Montgomery Prevention Council was formed in 1987 by concerned citizens from all three counties.
Originally named the Alcoholism and Substance Abuse Council of Hamilton, Fulton and Montgomery Counties, the mission of the council has always been to reduce the rate of substance abuse in our area.

In 2006 we changed our name to the HFM Prevention Council to help clarify the services and programs we offer. Cooperating with local agencies, we strive to provide quality educational programs, services and healthy alternatives to our counties’ youth and families.

Rob Constantine Recovery Community and Outreach Center

HFM Prevention Council operates a Recovery Community and Outreach Center to help prevent relapse and sustain long-term recovery. The Center facilitates recovery of individuals and their families by providing a substance-free environment. Included is a meeting location, substance-free social functions, prevention, and resource/referral center for services in the area. The center focuses on building the recovery community, and providing a positive outlook for individuals seeking help with addiction and throughout the recovery process.

Creative Connections Clubhouse

The Creative Connections Clubhouse provides equal access for all adolescents ages 12-17 and young adults 18-21, who have or are at risk of developing an addiction.

Education Programs

Educational programs that meet NYS education standards, are low cost or no cost, and are presented by trained prevention professionals.
